Outpatient cardiac rehabilitation programs are designed to help individuals with chronic cardiovascular disease to live well and reduce the risk of future cardiac events. The American Heart Association guidelines are used as the basis for healthy lifestyle recommendations following any hospitalization for heart disease. Fort Sanders Cardiac Rehabilitation has provided services since April 1, 1977. It is one of the first cardiac rehabilitation programs to receive national certification by the American Association of Cardiovascular and Pulmonary Rehabilitation for consistent clinical outcomes with heart patients. Fort Sanders Cardiac Rehabilitation is also aligned with two of the nation’s Top 100 cardiac hospitals that ensure quality care before, during and long after cardiac services are provided.
The nurses, exercise physiologists, dietitians, counselors, and pharmacists work as a team with physicians to provide an intensive lifestyle modification approach for heart disease management. The cardiac rehabilitation team will contact you in the hospital and again after discharge to help you understand instructions given by your cardiologist and surgeon.
Basic Information
Your doctor will tell you when you are ready to start cardiac rehabilitation. A nurse case manager from the Fort Sanders Cardiac Rehabilitation team will talk with you about your individual health goals when you enroll in the program. Participation is three sessions each week for 8-12 weeks depending on your progress toward these goals. Each session lasts for two hours with one hour for medically-supervised and monitored exercise and one hour for group education. Individual counseling on stress management, home exercise, diet and medications is also included. Most insurance companies cover cardiac rehabilitation services. Our nurse case managers will help with insurance approval and billing concerns. Our nurse case managers will also send reports to your referring doctor and your family doctor at the end of the program. Call (865) 531-5560 to schedule an intake appointment.
Spouses are welcome to attend education classes and observe exercise sessions at no charge.
Eligibility
Individuals are eligible for the Fort Sanders Cardiac Rehabilitation Program if they have heart disease and are referred by their attending physician.
